Registration for Services
  • Consumers 18 and over requesting county funded services need to contact the Lehigh County Information and Referral Unit by phone at 610-782-3200 or by clicking on the link.

  • Community Providers and hospitals requesting county funded services are to complete the referral form. Referral forms can be faxed to the AMH fax number listed on the bottom of the form. They can also be saved and attached to email to be sent to mentalhealth@lehighcounty.org

  • Priority for county funded services is given to those individuals with no insurance or  private insurance without mental health coverage.

  • Consumers seeking county funded services are required to come to the Government Center and complete a financial determination to see if the consumer will have a financial liability for services.  Consumers intake appointments are scheduled through Information and Referral.  At the time of intake, the consumer is required to bring proof of identification (photo preferred), Social Security card, proof of household income, which could include bank statements, social security award letters, if working; the last 2 pay stubs or any other documentation listing household income.

  • Consumers with Magellan Behavioral Heath who are only seeking outpatient services can either contact the Magellan member line at 1-866-238-2311 or click on the link to get a list of outpatient providers.
